Waiver of Liability

I, _________________________, agree to participate in the Great Lakes Demo Tour activities. I understand that I will be undertaking activities which inherently carry some risk of injury and property damage. I do release and agree to hold harmless the Great Lakes Demo Tour staff, sponsors, agents, assigns and all of their corporate sponsors, exhibitors and individuals those working in concert with them, from any and all liability associated with any acts or omissions involving my participation in the windsurfing and water sport activities. I represent to the Great Lakes Demo Tour staff that I am a swimmer, familiar with, and competent to participate in windsurfing and water sport activities and agree to bear all responsibility associated with any risk of injury.

In the event of injury to me, I grant to the Great Lakes Demo Tour staff the authority to act on my behalf and grant permission for my evaluation and treatment of any medical problems. I understand that should a major medical problem arise, an attempt will be made to notify my next of kin in person or by telephone. In the event they cannot be reached, I hereby give my consent to such treatment as may be deemed necessary.

I, _________________________, the parent or legal guardian of the minor child(ren), ______________________________________________________ hereby authorize my child(ren) to participate in the Great Lakes Windsurfing LLC windsurfing activities. I understand that my child will be undertaking activities which inherently carry some risk of injury and property damage. I do release and agree to hold harmless the Great Lakes Windsurfing, L.L.C., their members, sponsors, agents, assigns and all of the corporate sponsors, exhibitors and individuals working in concert with them, from any and all liability associated with any acts or omissions involving my child’s participation in the windsurfing and water sport activities. I represent to the Great Lakes Windsurfing LLC that my minor child is a swimmer, familiar with, and competent to participate in windsurfing and water sport activities. If my child is not an adequate swimmer familiar with water sport activities, I agree to participate directly in the supervision of my minor child and bear all responsibility associated with any risk of injury.
